ENT · Pharynx and tonsils: compartments of the parapharyngeal space
Sample itemA parapharyngeal abscess in the poststyloid compartment is more dangerous than one in the prestyloid compartment. Which structures lie in the poststyloid compartment?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
B. Carotid sheath contents and cranial nerves IX to XII
The styloid diaphragm divides the space into prestyloid (fat, deep lobe of parotid, branches of the mandibular nerve) and poststyloid compartments. The poststyloid compartment contains the internal carotid artery, internal jugular vein, vagus nerve and cranial nerves IX, XI and XII, plus the sympathetic chain. Abscess here may erode the carotid or thrombose the jugular vein.
Option by option
A. Thyroid gland and recurrent laryngeal nerve
The thyroid lies in the visceral compartment.
B. Carotid sheath contents and cranial nerves IX to XII (correct)
Correct. Great vessels and lower cranial nerves lie behind the styloid.
C. Parotid tail and retromandibular vein
These belong to the prestyloid compartment.
D. Sublingual gland only
The sublingual gland is in the floor of the mouth.
