ENT · Larynx and airway: vocal fold cyst
Sample itemA teacher has chronic hoarseness. Stroboscopy shows a smooth submucosal swelling of one vocal fold with a reduced mucosal wave that has not improved with voice therapy. What is the treatment?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
B. Microlaryngoscopic excision with a microflap technique
Vocal fold cysts are retention or epidermoid cysts in the lamina propria, which cause stiffness and hoarseness, and rarely respond to therapy alone. Surgical excision by microflap preserves the overlying mucosa and gives the best vocal outcome. Cordectomy is excessive.
Option by option
A. Radiotherapy
Radiotherapy is for malignancy.
B. Microlaryngoscopic excision with a microflap technique (correct)
Correct. Microflap excision preserves function.
C. Cordectomy
Cordectomy would remove the cord.
D. Oral antibiotics
Antibiotics do not treat cysts.
