Physiology · Reproduction: sexual differentiation

Sample item

A 46,XY boy has normal testes and male external genitalia, and a uterus and fallopian tubes are found at hernia repair. Which hormone or receptor is defective?

Original practice item, not a recalled exam question. Nothing here is recorded.

The answer and the reasoning

D. Anti-Mullerian hormone or its receptor

Sertoli cell anti-Mullerian hormone causes regression of the Mullerian ducts. If it or its receptor is defective, the uterus and tubes persist, while Leydig cell testosterone still masculinises the Wolffian ducts and the genitalia.

Option by option

  • A. The SRY gene product

SRY failure would prevent testis formation altogether.

  • B. Dihydrotestosterone formation

Lack of DHT would cause undervirilised external genitalia.

  • C. Testosterone synthesis

Deficient testosterone would impair Wolffian duct development and genitalia.

  • D. Anti-Mullerian hormone or its receptor (correct)

Correct. Failure of Mullerian regression with normal masculinisation fits AMH defect.

Questions about this Physiology question

What is the answer to this Physiology question?

The best answer is D, Anti-Mullerian hormone or its receptor. Sertoli cell anti-Mullerian hormone causes regression of the Mullerian ducts.

Why is "The SRY gene product" not the answer?

SRY failure would prevent testis formation altogether.

Why is "Dihydrotestosterone formation" not the answer?

Lack of DHT would cause undervirilised external genitalia.

Why is "Testosterone synthesis" not the answer?

Deficient testosterone would impair Wolffian duct development and genitalia.

What concept does this question test?

It tests Reproduction: sexual differentiation in Physiology. If it or its receptor is defective, the uterus and tubes persist, while Leydig cell testosterone still masculinises the Wolffian ducts and the genitalia.

How hard is this question?

We tag it as a multi-step question: it asks you to combine more than one idea. The tag is our own editorial judgement, not an official difficulty rating, so read the reason for every option, not only the answer.

How should I study Reproduction: sexual differentiation?

For each system, sketch the key graph (pressure–volume loop, oxygen dissociation curve, nephron handling) and explain it aloud. Then use the flashcards and the other Physiology questions on this site to test what stuck.

Is this a real NEET PG question?

No. It is an original practice item (editorial status: sample, version 1). NextPrep does not publish recalled or leaked questions, and independent faculty review of this item has not been completed yet, so confirm important facts in a standard textbook.
