OBG · Infertility: saline infusion sonography
Sample itemA woman with infertility and heavy bleeding has a thickened endometrium on routine ultrasound. Which test best distinguishes endometrial polyps and submucous fibroids?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
A. Saline infusion sonohysterography
Saline infusion sonography distends the cavity with saline during transvaginal scanning, outlining polyps, submucous fibroids, adhesions and septa, and is more accurate than standard scanning for intracavitary lesions. Hysteroscopy is the gold standard and allows treatment. HSG shows the cavity contour and tubal patency but not the myometrium.
Option by option
A. Saline infusion sonohysterography (correct)
Correct. Saline infusion defines intracavitary masses.
B. Serum AMH
AMH measures ovarian reserve and not cavity lesions.
C. Hysterosalpingography only
HSG is less sensitive for polyps and fibroid depth.
D. Semen analysis
Semen analysis assesses the male partner.
