Ophthalmology · Lids: congenital ptosis management
Sample itemA 3-year-old child has unilateral congenital ptosis with levator function of 2 mm. Which procedure is preferred?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
C. Frontalis sling
Poor levator function (under 4 mm) is treated by frontalis suspension using silicone rod or fascia lata. Levator resection needs fair function of 5 mm or more. Early surgery is done if the pupil is covered, to prevent amblyopia.
Option by option
A. Observation until age 15
Waiting risks deprivation amblyopia if the visual axis is covered.
B. Fasanella-Servat operation
Fasanella-Servat is for mild ptosis with good function such as Horner syndrome.
C. Frontalis sling (correct)
Correct. Frontalis sling is used when levator function is poor.
D. Levator resection
Levator resection needs a reasonable levator action.
