Ophthalmology · Lens: Endophthalmitis Vitrectomy Study
Sample itemA patient with post cataract surgery endophthalmitis has vision of perception of light only. According to the EVS, what is the management?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
A. Immediate vitrectomy with intravitreal antibiotics
The EVS showed that for vision of hand motions or better, tap and inject is adequate, but for light perception or worse, immediate vitrectomy gave better outcomes. Systemic antibiotics added no benefit. Intravitreal vancomycin and ceftazidime are used.
Option by option
A. Immediate vitrectomy with intravitreal antibiotics (correct)
Correct. LP-only vision needs immediate vitrectomy.
B. Vitreous tap and intravitreal antibiotics only
Tap and inject is enough for hand-motion or better vision.
C. Topical antibiotics alone
Topical antibiotics cannot reach the vitreous.
D. Systemic antibiotics alone
Systemic antibiotics were not beneficial in EVS.
